FILE OF LIFE
The File of Life card is a fast, simple and effective way for emergency medical personnel to obtain medical history when the patient is unable to offer one. During an emergency situation, an individual may not be able to give information because he/she may lose consciousness or be confused, and family members may be unable to provide all the pertinent information.
The card lists the following information needed for emergency care:
o Patient's name
o Emergency contacts
o Health plan policies
o Social security number
o Religion
o Doctors names
o Health care proxy
o Health problems
o Medications and dosages
o Allergies
o Recent surgeries
This information is kept in a magnetic, plastic 4x5 envelope labeled "File of Life" and placed on the refrigerator door. This makes it easily accessible for Emergency Medical Service.
Who is Service Available For?
Anyone who lives in the Montgomery ambulance service area can participate in the File of Life program by contacting the Montgomery Rescue. It is especially recommended for individuals who fall into any of the following categories:
o Has a history of heart problems, diabetes, high blood pressure or other diagnosed medical problems
o Takes prescription medication
o Lives alone
o Is a senior citizen (58% of all emergency calls are to seniors)
